This is probably a simple fix, but I merged the teeth into one object and reset the pivot point to be in the model’s centre. But whenever I try to mirror it, it seems to be off set, I’m not sure what I’m doing wrong; Can anyone help.

Probable reason is that you did not apply rotation before mirroring, It can cause some issues depending on how far from the pivot point is the geometry.
Yes rotation and scale need applying before modifier use.
