I think the extrusion of the holes is intersecting …
You are using Blender version 2.92. So new tool behaviours and or functionality changes are happening.
There are users who want to have the newest versions. but to follow the course it’s better to use the course version. Now you are experiencing bugs, unknow behaviour like your example.
