So i’ve just applied my multires modifier on 4 levels to my model. There is a hole going through the mesh and I cannot get rid of it. If i remesh or use dyntopo, it will destroy the details from the multires modifier. The sculpting tools won’t fix it. This is driving me insane

Basically, a vertex (or more) in the lower multi-res is managed differently by Blender. Can be part of a vertex group and or modifier. Difficult to spot.
Are you sure level 3 is correct!,
- Check the sequence of modifiers, or switch them off
- Select all faces unsmooth them, and smooth them again.
- No holes?
- All normals correct?
- You could remove vertices in that area when these are not part of the curvy flows. (Simply fy mesh)

I would mute all the modifiers, then see if you can repair the basic mesh.
Save first obviously.
