I tried just the spot, just the emissive mesh and both together. I liked the look of the emissive most but the spot produced a less spotty (d
uh) render and still looked OK with both together. I then wondered about the effect of a white reflective surface inside the lamp and added a mesh shade with white material (shown here). I was disappointed to see that the result was the most spotty of all options. Maybe a direct ray path is cleaner?

Try it out and check also the render times!
When animated some of the problems can not be seen due to the animation flow and speed.

Are you not using denoising? Should clean that up easily.
