I get this problem when I use the mirror modifier. I even enabled the merge and clipping in the mirror modifier.

What is the problem?
Is it that you have pushed the verts that should not get close together right to the mirror line and made them clip together?
Yours on the left. Correctly on the right. You do not want to join those second in verts.

Turn down the clipping value in the mirror modifier to prevent them “over clipping” and snapping to the nearest vertex at the centre
